Location: Brighton and Hove with travel around the area
Hours: 21
Salary: £24,479 per annum pro-rata
Contract type: Permanent
Driving Requirement: A full UK driving license is required and access to a vehicle. Business mileage reimbursed.

We are currently seeking dedicated and compassionate individuals to join our team as a Support Worker- Home from Hospital in East Sussex. This is an excellent opportunity to contribute to the well-being and independence of individuals within our community.
What will a day in the life of a Support Worker involve?

Managing a caseload to provide emotional and practical support for our service users who have been discharged from hospital including welfare calls, shopping, and home visits.

Supporting adults after a hospital stay to help them recover safely at home.

Responding to referrals from clinicians and community health professionals.

Assessing needs and following a personalised support plan for each service user.
To be a successful Support Worker, what will you need?

A kind, calm and confident approach with great people skills.

The ability to work independently and as part of a team.

A full UK driving licence and access to a vehicle

Basic IT skills and a commitment to following procedures.
